This is a "Nejishiki" (Screw Style) T-shirt by Yoshiharu Tsuge.

Purchased about 25 years ago through mail order from Seirindo's monthly magazine "Garo."
It is a new, unused item.
It is still in the original plastic bag from when it was purchased.
(I have not checked the condition of the inside)
Limited to 500 pieces, lot number 127.
As shown in the third photo, there is a little black ink-like mark on the bag.
